How do I email the collected form information back to me via the submit button?

You need to use a PHP or ASP script on your server. An example of a form processor for simple form e-mail is in the Tutorial under the Using Forms->Form mail scripts section.

You may also find that most web hosts have their own help sections, sample scripts, and how to set them up.
