2. Multi-page (or multi-part) forms require some sort of script in order to move between the different form pages. There would also need to be a method to host the multiple pages on a single work page. Most multi-page scripts I have come across are written in PHP, and a few in javascript.

Have you tried using a single form spaced such that it can be broken into "separate pages" and then displayed inside an i-frame which has been sized to show only a single "page" of the form? Movement between the pages is controlled via page anchors. I have put together a very simple demo here.
